You know that if you get money for your birthday, you're just going to blow it on useless junk. You would rather have a gift card to your favorite stores so you will have to go out and pick out something that you're going to use.
Clothes
Your wardrobe is either seriously outdated, or you just love clothes. When someone asks you what you want for your birthday this year, you're going to want to get specific, so you don't end up having to take a bunch of stuff back.
Money
There are bills to pay, and you don't have the money to do it right now. You would much rather have the cash to toss towards your responsibilities than some gift you aren't going to be able to use. It's okay to ask for money when people want to know what to get you. It's easier for them anyways!

There isn't anything that anyone could buy you that would you can't get for yourself. The thought of someone spending their money on you probably makes you a little uncomfortable too. Maybe you are too old for birthday gifts. Whatever the case is, you can honestly tell people nothing when they ask you what you want this year.
